Configure the Internal Request Form

Customize the internal request form to control what information UpKeep users provide when submitting work requests.

The Internal Request Form is used by users with UpKeep licenses to submit work requests directly within UpKeep.


How to Edit the Internal Request Form

  1. Navigate to Requests from the left-hand menu

  2. Click the three dots next to "Create Request"

  3. Select Edit Request Form

  4. Ensure you are viewing Internal Request

NOTE: You can also locate the Internal Request Form by navigating to Settings → Requests → Internal Request Form.


Configure Field Permissions

For each field, choose:

  • During Create – What requesters see when submitting

  • During Approval – What admins see when reviewing

Set each field to:

  • Required

  • Optional

  • Hidden


Add Custom Fields

You can add custom form items to collect additional details. Responses to these items will populate the request description and carry over to the work order once the request is approved.


Save Changes

All changes are automatically applied—no save button needed.
