Manage Your Request Forms

With UpKeep, you can configure Request Forms to collect the right information upfront—so your team can create faster, more effective Work Orders.

Why Request Forms Matter

Request Forms help standardize how work is submitted by:

  • Ensuring required details are captured

  • Reducing back-and-forth communication

  • Improving work order quality and efficiency

You can customize fields by making them:

  • Required – Must be filled out

  • Optional – Can be filled out if needed

  • Hidden – Not visible to the user

You can also add custom questions to gather additional information.


Work Requests vs Work Orders

Work Requests are not Work Orders.

  • Work Requests must be reviewed and approved

  • Only Admins or Limited Admins can approve requests

  • Once approved, they become Work Orders


Types of Request Forms

UpKeep offers 3 types of request forms:

Internal Request Form

  • Used by users that have an UpKeep login

  • Accessible within the UpKeep app

New Request Portal

  • Used by external users that do not have an UpKeep user license

  • Accessible via a shareable link or QR code

Legacy External Request Form

  • Used by external users that do not have an UpKeep user license

  • Accessible via a shareable link or email

  • This is a legacy feature and is no longer being updated or enhanced

Can I use both internal and external forms?
Yes—both can be used at the same time.

Can requesters see other users' requests?
No—requesters can only see their own submissions.

Can View-Only users submit requests?
Yes.

Can I update request forms anytime?
Yes—changes apply immediately to new requests.

Can multiple people share the same requester email?
Yes, but all notifications will go to that email address.

Who can use the Request Portal?
The Request Portal is available on Professional and Enterprise plans.
