The Lockout Tagout (LOTO) module in UpKeep EHS is used to digitally manage energy isolation during maintenance or servicing work. It helps ensure that all energy sources are properly identified, isolated, and verified before maintenance begins. Using LOTO procedures helps protect workers from the unexpected release of hazardous energy and supports compliance with safety regulations.


Creating a LOTO Document

  1. Start with AI Assistance

    • Click +Create LOTO

    • You can describe your task using voice or text (e.g., “Create a lockout tagout for replacing the hydraulic pump on Line 2”).

    • The AI will auto-generate a draft including title, description, and relevant placeholders

  2. Energy Source Identification
    This section is crucial for safety. Each identified energy source should include:

    • Type of Energy (e.g., electrical, hydraulic, pneumatic)

    • Isolation Point (location where energy can be disconnected)

    • Isolation Method (valve, switch, disconnection, etc.)

    • Lockout Device (padlock, tag, hasp, etc.)

    • Verification Method (how to confirm zero energy)

    • Additional Notes (optional)

    You can add or delete energy sources as needed

  3. Procedure Steps
    The document is divided into three sections:

    • Pre-Procedure Requirements: Preparations before isolation begins

    • Main Procedure Steps: Step-by-step isolation process

    • Zero Energy Verification: Confirming that no energy is present before work starts (mandatory)

    • Post-Procedure Requirements: Steps to safely restore energy or remove locks/tags

    You can rearrange, edit, or expand any step, attach photos, videos, or documents, and use the AI Writing Assistant to refine content.

  4. Safety Protocols
    This section captures emergency procedures and contact information, including:

    1. Emergency Contacts

    2. First Aid Response

    3. Group Lockout Procedure: Defines roles and responsibilities for team-based lockouts. A Group Lockout Coordinator must verify all devices and maintain the lockout log

    4. Shift Handover Procedure: Required when work extends across shifts. Details should include lockout status, outstanding steps, equipment condition, and transfer of documentation

  5. Metadata & Approvals

    • Document Owner, Permit Holder, and Authorizing Personnel

    • Next Review Date for scheduled audits

    • Linked Location and Asset

    • Approval Workflow: Choose Sequential (step-by-step) or Parallel (all at once) approvals.

    • Digital Sign-off: Each approver can electronically sign and approve.
