The OSHA Log (Form 300) is used to record work-related injuries and illnesses. In UpKeep EHS, completing this log ensures accurate reporting and feeds directly into your Form 300A Summary and Agency Reports.
Step 1: Access the OSHA Log
In the left-hand navigation, go to OSHA > OSHA Log (Form 300).
Click + Create OSHA Record to begin a new entry.
Step 2: Link a Safety Event
Select an existing Safety Event to associate with this OSHA record. This is a required step and ensures the incident is traceable throughout the EHS platform.

Step 3: Fill Out Employee Information
Enter information about the employee involved in the incident:
Employee Name (required)
Job Title
Work Location
Department
Date of Hire
Shift
Supervisor Name
OSHA Location (required)
Optional: Mark as Privacy Case
If the incident involves sensitive personal or medical information, toggle the Mark as Privacy Case option to restrict visibility of the record.
Step 4: Provide Medical Information
Record details about the injury or illness:
Body Part Affected
Type of Injury
Treatment Location
Type of Medical Care:
First Aid
Medical Treatment
Emergency Room
Overnight Hospital Stay
Prescription Medication Given (optional checkbox)
Step 5: Complete OSHA-Specific Questions
OSHA requires additional information to determine if an incident is recordable:
Was the employee hospitalized as an in-patient?
Did the incident result in death?
Days Away From Work
Days of Restricted Work Activity
Step 6: Enter Witness and Reporter Details
List the individuals who witnessed or reported the incident:
Witnesses (comma-separated names)
Reported By (defaults to safety event reporter; can be edited)
Step 7: Add Corrective Actions
Document your organization's response and preventive measures:
Root Cause Analysis
Corrective Actions
Step 8: Finalize OSHA Reporting Section
Select the appropriate classification for the incident:
Primary Recordable Outcome (required)
Reason for Reportability (optional text field)
Completing this section will automatically update your OSHA 300 Log and Form 300A Summary.

Step 9: Save and Retain the Record
Click Save OSHA Form to submit the entry. A banner will appear with the following notice:
This form must be retained for five years from the end of the calendar year it covers. Completing this form automatically updates your OSHA 300 Log and 300A Summary.
