What is a CAPA?
A CAPA (Corrective and Preventive Action) is used to document and resolve the root cause of an incident. In UpKeep EHS, you can create CAPAs linked to incident reports to track specific corrective steps and long-term preventative measures that reduce risk and improve safety.
How to Create a CAPA
Go to the CAPAs Tab
From the left-hand menu, click CAPAs
Click “Create”
Use the + Create button in the top-right corner
Fill in Basic Details
Complete the Root Cause Analysis
Voice Input (optional): Use the AI Assist button to record a voice explanation
RCA Method Used: Select from options like 5 Whys, Fishbone Diagram, Fault Tree Analysis, etc.
RCA Findings & Conclusion: Summarize your investigation and reasoning
Root Cause Category: Choose from:
Human Error
Equipment Failure
Environmental
Procedural
Other
Propose Actions
Describe both immediate corrective actions and long-term preventive steps
Add supporting files or media (up to 5 attachments)
Assign and Prioritize
Owner: Assign the CAPA to a responsible team member
Due Date: Select when the CAPA should be completed
Priority: Choose Low, Medium, or High based on severity or urgency
Add Tags (Optional)
Select relevant tags such as Training, Policy, Hazard, Equipment, Procedure, or Personnel to support tracking and filtering
Visibility Settings
Check Private to Administrators if this CAPA should not be visible to general users
Click “Create CAPA”
Your CAPA is now saved and ready to be managed in the CAPA dashboard
Tips: Root Cause & Voice Recorder
🎤 When should I use the voice recorder during Root Cause Analysis?
The voice recorder is designed to help you summarize your RCA findings—not conduct the investigation itself. Use it after you’ve completed your root cause analysis (e.g., 5 Whys or Fishbone).
✅ Summarize, don’t investigate: Speak your key findings and conclusions after analysis is complete.
🧠 AI-enhanced: The system detects terms like "5 Whys" or "Fishbone" and will auto-select the appropriate RCA Method for you.
📎 Upload optional: If you created your RCA externally (like a diagram or worksheet), you can attach it as a file instead.
💡 Bonus: The AI may suggest corrective actions based on what you say during the recording.
📌 Tip: Think of the recorder as a structured note-taker—it captures and enhances what you already discovered.
