Custom Safety Templates

Custom Safety Templates allow you to create safety report forms that reflect how safety is managed in your organization. Instead of relying on a single standardized form, you can design templates that capture the exact information required for different safety scenarios.

This article explains how to create, customize, and use Custom Safety Templates.


What Are Custom Safety Templates?

Custom Safety Templates are configurable safety report forms that can be tailored to your organization’s processes. While core safety fields are included by default, all additional fields can be added, removed, or modified to meet your needs.

Templates can be assigned to access points, allowing different locations or scenarios to use different safety forms.


Create a Safety Template

  1. Select Settings

  2. Navigate to Safety Templates

  3. Click Create Template

  4. Enter a template name

  5. Customize your Form

  6. Select Save


Default Required Fields

Each safety template includes the following required fields:

  • Title

  • Date & Time

  • Description

These fields cannot be removed. All other fields are optional and fully customizable.


Add Fields to a Template

Fields can be added to the form canvas by dragging and dropping them or selecting the plus (+) icon.

Standard Field Types

You can add common field types such as:

  • Short text

  • Long text

  • Number

  • Dropdown

  • Multi-select checkboxes

  • Date & time

  • File upload

  • GPS location

Safety-Specific Field Groups

To streamline form creation, pre-configured safety field groups are available:

  • Incident Details – Capture information related to safety incidents

  • Injury & Medical – Document injuries or medical events

  • Cause & Risk Analysis – Record contributing factors and risks

  • Environmental & Property Impact – Track environmental or property damage

  • Guest, Customer & Legal – Capture information related to non-employees

  • Sign-Off & Review – Require review or approval from a supervisor

  • Diagrams & Sketches – Add body diagrams, accident layouts, or freeform drawings

These field groups help ensure important safety information is consistently collected.


Edit Field Settings

Select any field on the canvas to open Field Settings.

From here, you can:

  • Update the field label

  • Add help text

  • Mark the field as required

  • Delete the field

Note: Some fields, such as multi-select fields, require at least two options before they can be saved.


Add Conditional Logic

Use Conditional Logic to show/hide fields or change validation (like making a field required) based on how someone fills out the form.

  1. Open your safety template in the builder.

  2. Select the Logic tab (next to Fields)

  3. Click Add Rule

  4. In the rule builder, configure:

    • When this field: choose the field you want to evaluate (example: Title)

    • Operator: choose how to match (example: Contains)

    • Value: enter the value to look for (example: Fire)

    • Then: choose the action (example: Require)

    • This field: choose the field affected by the rule (example: Fire Safety Check)

  5. Click Add Rule to save the rule

Example

  • When Title Contains Fire

  • Then Require Fire Safety Check


Save and Preview a Template

Click Save Changes at any time to update the template.

Select Preview to view the form as users will see it. Preview mode allows you to verify required fields, layout, and behavior before assigning the template.

Safety templates are supported on both desktop and mobile devices.


Assign a Safety Template to an Access Point

After creating a template, it can be assigned to an access point.

  1. Create or edit an Access Point

  2. Select the Event Form Template dropdown

  3. Choose the desired safety template

  4. Generate the QR code

When the QR code is scanned, users will see the selected safety template and all required fields configured in the form.


Additional Notes

  • Multiple safety templates can be created and used across different locations or scenarios.

  • Templates can be updated at any time. Changes apply to future submissions.
