Step-by-Step Guide

  1. Access the OSHA Log
    In UpKeep Safety, go to OSHA > OSHA Log (Form 300) and click + Create OSHA Record.

  2. Link a Safety Event & Enter Employee Information
    Select the corresponding Safety Event, then complete employee details (Name, Job Title, Department, OSHA Location).

    Toggle Privacy Case if sensitive personal details are involved.

  3. Specify Medical & Injury Classification Fields
    Select the Injury/Illness Type (Injury, Skin Disorder, Respiratory Condition, Poisoning, Hearing Loss, All Other Illnesses) and specify the Object/Substance that directly caused or contributed to the event.

  4. Record Days Away & Outcome
    Enter Days Away From Work and Days of Restricted Work Activity. Select the Primary Recordable Outcome.

  5. Save & Export PDF/CSV
    Click Save OSHA Form. You can now download full OSHA 300 PDF logs or export CSV files containing all enhanced injury type and object/substance data.

    UpKeep does not automatically send OSHA reporting, you must download the PDF and submit it yourself.
